Privacy Statement

  

Parent to Parent of NYS is dedicated to maintaining the privacy and health information that individuals disclose to coordinators of our programs. 

 

We understand that information about an individual and their family is personal.  We will  safeguard and respect the privacy of individuals and their families.

 

We will protect all information that is provided to Parent to Parent including names, addresses, birth dates and medical information.

 

Parent to Parent will disclose information only with specific permission from a parent or guardian of a child or when mandated by law.  

 

Disclosures mandated by law may include the following instances:

1.      Public Health Risks or threats to health or safety including reporting child abuse or neglect.

2.      Law Enforcement - We may release identifiable health information in response to a summons, warrant, court order or similar legal process; in an emergency to report a crime.

 

An individual has the right to confidential communication and the right to request that our office communicate at a particular location or in a particular manner.

 

Any authorization provided to us regarding the use and disclosure of identifiable health information may be revoked at any time.

 

If an individual believes their privacy has been violated, they may file a complaint.  To file a complaint with our organization, contact Janice Fitzgerald, Executive Director, Parent to Parent of NYS, P.O. Box 1296, Tupper Lake, NY 12986.
